For some reason or another, the machine did not drill and tap the last two holes.

You will have to re-drill and re-tap (not really recommended due to the hardness-you will probably break the tap)

or find an alternative. It would be possible to tack weld the fourth clasp on.

Also the machine did not debur the bottom edge-easy fix with a dremel or file.
